CALLING ALL GERIATRIC SOCIAL WORKERS! Do you love older adults but feel beaten down by paperwork and the constraints of working in a facility? Do you go home from work wishing you had more time to spend with that resident who just needed a little extra TLC today? As an Eldercare Navigator, you get the chance to create meaningful relationships with clients, spending the time they need, on a schedule that works for you. The current position is flexible in terms of how many clients you take, and how many hours you work. However, availability is the key, so being available Mon-Fri is essential in order to accommodate healthcare appointments.


Our Care Managers are called Eldercare Navigators, because it explains clearly what the role is. An Eldercare Navigator acts as a liaison between the client, their family members, and healthcare providers, with the goal of improving quality of life and continuity of care. The primary objective is to ensure your client's voice is heard so all decisions reflect their values. This position requires a creative approach to problem solving, out-of-the-box thinking, patience, and sensitivity. You'll need to juggle complex family dynamics, advocating for client wishes, navigating transitions between home and facilities, and managing client expectations.

Who We Are Looking For:

We want social workers who love working with older adults and have experience in the long term care continuum. You must have a firm grasp of the complexities related to aging and the sensitivity to know how to broach emotionally charged subjects. You need to address immediate needs while anticipating future needs by creating plans to mitigate potential issues. Our team members are kind, open, confident, resilient, willing to learn and grow, trustworthy, patient and at times you will need thick skin.


What You Do:

Navigators travel to see clients in their homes, accompany them to appointments, create mature life plans, and provide guidance and support to ensure every person is living their best life according to their values. Your approach is holistic and client-centered with a focus on managing needs in crisis and creating proactive plans which anticipate future needs. This is done through assessment, planning, problem-solving, monitoring, education and advocacy.

  • You will train with us at our main office in Somerville, accompanying Navigators on client visits to understand the culture and values essential to being a Nightingale Navigator. Once you are seeing clients independently your schedule is flexible, meeting clients at times that work for you together. 
  • As a Navigator, you control how many clients you can manage. Although you become the primary Navigator for specific clients, you always have the support of our team, and a back-up colleague who knows your clients and can assist when you are not available. 
  • Weekly 1:1 and monthly team meetings ensure you always feel supported, and provide opportunities for education and growth. NightingaleNJ’s mission, “Your Voice. Heard.” applies to our team as well as our clients.
